Please ship to Vidium Animal Health at 445 N 5th Street, Phoenix, AZ 85004. It is preferrable to have samples sent directly to Vidium in order to expedite services.

If you will be sending a sample from a pathology lab and prefer Vidium acquire that same sample, please provide pathology information and we will do our best to coordinate shipping of your sample from the pathology lab to us for an additional $100 fee that is a pass-through cost of the pathology and shipping charges Vidium will incur.
